By taking part in a tourist trip organized by AroGlobe Pro Services, Inc., I declare that:

1. I have read the terms and conditions of participation in the tourist trip, including the program, costs, schedule, safety rules and regulations of the organizer of the trip. I agree to comply with the rules and instructions of the organizer regarding safety and order during the trip.

2. I agree that AroGlobe Pro Services, Inc. is not responsible for any accidents, injuries, contusions, death or material losses that may occur during the trip, regardless of the cause, including as a result of force majeure, actions of other participants of the trip or accidental events. The office is not responsible for changes in the trip program, changes in travel routes, flight delays, changes in weather conditions or other unforeseen circumstances that may affect the course of the trip.

3. I declare that before starting the trip, I informed the organizer of any existing health problems that may affect my participation in the trip or require specialist medical care. The travel agency is not responsible for the deterioration of my health, the occurrence of chronic diseases, allergies, injuries or other health problems that may occur during the trip. I undertake to take medication regularly, if necessary, and to follow the recommendations of doctors regarding my health during the trip.

4. I declare that I have valid travel insurance that covers the costs of treatment, hospitalization, possible repatriation and assistance in the event of an accident or illness during the trip. The travel agency is not responsible for the lack of insurance or for any costs related to treatment that may occur during the trip.

5. I agree that AroGlobe Pro Services, Inc. is not responsible for changes in the trip program resulting from circumstances beyond the agency's control, such as flight delays, strikes, changes in weather conditions, decisions of local authorities or other unforeseen situations.

6. I consent to the use of my image, photos and film materials taken during the trip for promotional purposes of AroGlobe Pro Services, Inc. In the event of an objection, I am obliged to inform the organizer before the start of the trip.

7. I declare that in the event of any existing health problems that may affect my safety or comfort during the trip, I have informed the organizer before the start of the trip.

8. I agree that I am liable for any damage caused by me during the trip, both material and health, including damage to property, equipment, and harm to other participants of the trip.

9. I consent to the processing of my personal data by AroGlobe Pro Services, Inc. for the purpose of organizing the trip in accordance with the provisions on personal data protection.

Biorąc udział w wyjeździe turystycznym organizowanym przez AroGlobe Pro Services, Inc. oświadczam, że:

1. Zapoznałem/am się z warunkami uczestnictwa w wyjeździe turystycznym, w tym z programem, kosztami, harmonogramem, zasadami dotyczącymi bezpieczeństwa i regulaminem organizatora wyjazdu. Zgadzam się na przestrzeganie zasad i poleceń organizatora w zakresie bezpieczeństwa oraz porządku podczas wyjazdu.

2. Zgadzam się, że AroGlobe Pro Services, Inc. nie ponosi odpowiedzialności za żadne wypadki, urazy, kontuzje, śmierć, ani straty materialne, które mogą wystąpić w trakcie wyjazdu, niezależnie od przyczyny, w tym w wyniku sił wyższych, działań innych uczestników wyjazdu lub przypadkowych zdarzeń. Biuro nie ponosi odpowiedzialności za zmiany w programie wyjazdu, zmiany tras podróży, opóźnienia lotów, zmiany warunków atmosferycznych, ani inne nieprzewidziane okoliczności, które mogą wpływać na przebieg wycieczki.

3. Oświadczam, że przed przystąpieniem do wyjazdu poinformowałem/am organizatora o wszelkich istniejących problemach zdrowotnych, które mogą wpłynąć na moje uczestnictwo w wyjeździe lub wymagają specjalistycznej opieki medycznej. Biuro turystyczne nie ponosi odpowiedzialności za pogorszenie mojego stanu zdrowia, wystąpienie chorób przewlekłych, alergii, urazów ani innych problemów zdrowotnych, które mogą wystąpić w trakcie podróży. Zobowiązuję się do regularnego przyjmowania leków, jeżeli jest to konieczne, oraz do przestrzegania zaleceń lekarzy dotyczących mojego stanu zdrowia podczas wyjazdu.

4. Oświadczam, że posiadam ważne ubezpieczenie turystyczne, które obejmuje koszty leczenia, hospitalizacji, ewentualnej repatriacji oraz pomoc w przypadku wypadku lub choroby w trakcie wyjazdu. Biuro turystyczne nie ponosi odpowiedzialności za brak ubezpieczenia ani za wszelkie koszty związane z leczeniem, które mogą wystąpić podczas podróży.

5. Zgadzam się, że AroGlobe Pro Services, Inc. nie ponosi odpowiedzialności za zmiany w programie wyjazdu wynikające z niezależnych od biura okoliczności, takich jak opóźnienia lotów, strajki, zmiany warunków atmosferycznych, decyzje władz lokalnych, czy inne nieprzewidziane sytuacje.

6. Wyrażam zgodę na wykorzystanie mojego wizerunku, zdjęć oraz materiałów filmowych wykonanych podczas wyjazdu w celach promocyjnych AroGlobe Pro Services, Inc. W przypadku sprzeciwu, jestem zobowiązany/a poinformować organizatora przed rozpoczęciem wyjazdu.

7. Oświadczam, że w przypadku jakichkolwiek istniejących problemów zdrowotnych, które mogą wpłynąć na moje bezpieczeństwo lub komfort podczas wyjazdu, poinformowałem/am o tym organizatora przed rozpoczęciem wyjazdu.

8. Zgadzam się, że ponoszę odpowiedzialność za wszelkie szkody wyrządzone przeze mnie w czasie trwania wyjazdu, zarówno materialne, jak i zdrowotne, w tym uszkodzenie mienia, sprzętu, a także szkodzenie innym uczestnikom wyjazdu.

9. Wyrażam zgodę na przetwarzanie moich danych osobowych przez AroGlobe Pro Services, Inc. w celu organizacji wyjazdu zgodnie z przepisami o ochronie danych osobowych.
